1. Agreement to Terms

These Terms of Service ("Terms") govern your use of the Jerry Skyles Therapeutic Massage website located at this domain and your receipt of massage therapy services provided by Jerry Skyles ("Therapist," "we," "our," or "us"). By accessing our website or scheduling an appointment, you agree to be bound by these Terms. If you do not agree, please do not use our services or website.

2. Services

Jerry Skyles Therapeutic Massage provides licensed massage therapy services in Portland, Oregon, including but not limited to Swedish relaxation massage, deep tissue therapy, heated stone massage, injury recovery massage, reflexology, and energy work. All services are performed by Jerry Skyles, a Licensed Massage Therapist (LMT) licensed in the State of Oregon.

3. Appointments and Scheduling

Booking

Appointments may be scheduled by calling (503) 757-7295 or by emailing jselyks@aol.com. An appointment is confirmed only when you receive direct confirmation from us.

Arriving for Your Appointment

New clients are asked to arrive 10–15 minutes early to complete a health intake form. Arriving late may result in a shortened session, as subsequent appointments must begin on time. The full session fee applies regardless of late arrival.

Cancellation Policy

We require a minimum of 24 hours' notice to cancel or reschedule an appointment. Cancellations made with less than 24 hours' notice, or failure to appear for a scheduled appointment without notice, may be subject to a cancellation fee equal to 50% of the scheduled service fee. We understand emergencies happen and will consider extenuating circumstances on a case-by-case basis.

4. Payment

Payment is due at the time of service unless prior arrangements have been made. We accept cash, check, and major credit cards. Prices for services are as displayed at the time of booking and are subject to change without notice. We will notify you of any price changes prior to your scheduled appointment.

We accept some insurance plans. Coverage must be confirmed in advance of your appointment. We are not responsible for insurance claims that are denied and payment remains your responsibility in such cases.

5. Health and Medical Disclosure

Prior to your first session, you will be required to complete a health intake form disclosing any known medical conditions, injuries, medications, or contraindications. You agree to provide complete and accurate health information and to update us of any changes at subsequent visits.

Massage therapy is not a substitute for medical care. We do not diagnose medical conditions or prescribe treatments. If you have a medical condition, you are encouraged to consult your physician before receiving massage therapy.

Certain conditions may be contraindicated for massage, including but not limited to: fever, infectious illness, open wounds, blood clots (DVT), uncontrolled hypertension, certain skin conditions, and recent surgery. We reserve the right to refuse or modify service if we believe it poses a risk to your health or safety.

6. Professional Conduct

Jerry Skyles Therapeutic Massage is a professional therapeutic practice. All sessions are conducted in a professional, respectful manner in compliance with the Oregon Board of Massage Therapists' standards of practice and code of ethics.

Sexual misconduct of any kind is strictly prohibited. Any behavior of a sexual nature — verbal, physical, or otherwise — will result in immediate termination of the session, full payment will be due, and the matter may be reported to appropriate authorities.

We reserve the right to refuse or discontinue service to any client who is under the influence of alcohol or drugs, behaves in a threatening or disrespectful manner, or for any other reason that compromises the safety or professionalism of the session.

7. Age Requirements

Clients under the age of 18 must have written consent from a parent or legal guardian and the parent or guardian must be present during the session. We do not provide services to minors under the age of 16 without prior arrangement and documented parental consent.

8. Limitation of Liability

To the fullest extent permitted by applicable law, Jerry Skyles Therapeutic Massage shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services or website. Our total liability to you for any claim arising from our services shall not exceed the amount paid by you for the session in question.

While we take every reasonable precaution to ensure your safety and comfort, massage therapy involves inherent physical contact and carries some degree of risk, including temporary soreness, bruising, or discomfort. By scheduling an appointment, you acknowledge these risks and agree to hold us harmless from claims arising from the ordinary and reasonable provision of massage therapy services.

9. Website Use

This website is provided for informational purposes only. You agree not to use the website in any way that is unlawful, harmful, or that could damage, disable, or impair the website or interfere with any other party's use. We reserve the right to modify, suspend, or discontinue the website at any time without notice.

Content on this website, including text, images, and design, is owned by or licensed to Jerry Skyles Therapeutic Massage and may not be reproduced, distributed, or used without prior written permission.

10. Reviews and Submissions

By submitting a review through our website, you grant Jerry Skyles Therapeutic Massage a non-exclusive, royalty-free, perpetual license to display, reproduce, and use your review for promotional or informational purposes. You represent that the content you submit is truthful, does not infringe any third-party rights, and does not contain unlawful, defamatory, or offensive material. We reserve the right to remove any review at our discretion.

11. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Oregon, without regard to its conflict of law provisions. Any dispute arising under these Terms shall be resolved in the state or federal courts located in Multnomah County, Oregon, and you consent to personal jurisdiction in those courts.

12. Changes to These Terms

We reserve the right to update or modify these Terms at any time. The "Effective Date" at the top of this page reflects the date of the most recent revision. Continued use of our website or services after changes are posted constitutes your acceptance of the revised Terms. We encourage you to review these Terms periodically.
